FC Green Bay Spirit Wear order procedure.
1. Print form and make selections of items wanted, clearly mark sizes and quantities wanted.
2. Write Player's Name, Phone # and, Team age, gender as well as color in the designated box at the bottom of form.
3. Write the check # in the box or write Cash, if payment in cash. Payment is due at time of order (NO EXCEPTIONS). Please attach the check or cash to the order, use paperclip!
4. Mail or give the order + payment to your team manager or representative. (they will submit one total order for your team!)
5. The distribution of items will de done through your team manager or representative.
